Drug & Alcohol Awareness course booking form

This is an online course booking form. Submission of this form does not guarantee you a place on the course. Confirmation of course places are issued approximately four weeks before the course start date. However, if you do not hear from us at least 7 days before the course date, please call the relevant course administrator direct or 0208 489 6909.

Please note that there is a charge of £50 if you do not live and/or work within Haringey, or if do not attend a course you'd booked (unless you give at least 48 hrs notice of cancellation).

Once you have submitted the form, you will be given an application logging number and a summary of your submitted data that can be printed out for future reference. An acknowledgement email of your booking will be sent to you on approval from your manager.
